At Berry Street, our top priority is supporting providers to build thriving in-network practices. One of the easiest ways to increase your income and reach is to acquire additional licenses in new states!. More states = more patients who can book with you. To help you expand your reach, and make getting a new state license just a little easier, we’ve outlined the process for each state.

We’ve created this guide to break down the application process for each and every state, step-by-step.

For a limited time, we’ll cover your licensing fees for the states listed below. Please note you do need to be accepting Berry Street clients in order to qualify for this program.

#### **Here’s how it works:** <a href="#heres-how-it-works" id="heres-how-it-works"></a>

**What:** We’ll reimburse your state licensing application fees — up to **$250** per license! **Where:** Available in all states except AZ, CA, CO, NJ, MI, VA, CT, MA, NY, TX

**Who’s Eligible:** Dietitians actively accepting clients on Berry Street. **When:** Submit your documentation, and you’ll be reimbursed within **60 days**.

#### **To qualify, complete the following steps:** <a href="#to-qualify-complete-the-following-steps" id="to-qualify-complete-the-following-steps"></a>

**Step 1:** Once your new license is active, email the following to <help@berrystreet.co>:

* Your full name
* The email address you use with Berry Street
* A photo or PDF of your new license (include license #, issue & expiration dates)
* The amount you're requesting for reimbursement
* Copies of receipts showing the licensing fee(s)

**Step 2:** [Complete this form](https://web.miniextensions.com/3DVtHyF9O8qxWpx4s1zk) so we can update your CAQH profile.

* Reimbursement is for **new licenses only** (not renewals).
* You must remain available for new Berry Street clients for **at least 60 days** after your license is active.
