How to Apply & File:

To get temporary credential while permanent credential is pending:

  1. Create an account, select the desired license (Dietitian, Certified), choose “Act 10” application, complete the application, and pay the fee online here.

To get permanent credential by reciprocity:

  1. Submit dietitian application and pay appropriate fee online here.

  2. Submit letters from all State Boards where credentialed/certified, active and inactive.

  3. Submit info on Convictions and Pending Charges (if applicable).

  4. Submit info on Malpractice Suits or Claims (if applicable).

$70 ($10 temporary dietitian certificate fee, $60 credential fee)

After You Apply - What to Expect

  • When can I start practicing?

    • A healthcare provider must apply for a temporary credential within 30 days of beginning to provide services in Wisconsin for a health care employer.

    • Dietitians are able to practice with this temporary credential while awaiting permanent credential.

Renewal

The required renewal date for the Dietitian credential is 10/31 of each even-numbered year.
