To get temporary credential while permanent credential is pending:
Create an account, select the desired license (Dietitian, Certified), choose “Act 10” application, complete the application, and pay the fee online here.
To get permanent credential by reciprocity:
Submit dietitian application and pay appropriate fee online here.
Submit letters from all State Boards where credentialed/certified, active and inactive.
Submit info on Convictions and Pending Charges (if applicable).
Submit info on Malpractice Suits or Claims (if applicable).

When can I start practicing?
A healthcare provider must apply for a temporary credential within 30 days of beginning to provide services in Wisconsin for a health care employer.
Dietitians are able to practice with this temporary credential while awaiting permanent credential.
