2. Complete Criminal Records Check

Licenses are valid for 1 year from the date of issuance.

How to Renew:

   1. Licensed Dietitians, Respiratory Therapists and Polysomnographic Technologists are required to take the Oregon Health Authority's Health Care Workforce Survey online at the time of their renewals. These surveys are now integrated with the online process, you will be directed to complete the survey before returning to submit the fee payment.
