How to Apply & File:

Online:

  1. Create a Secure Access Washington Account.

    1. Select “Create One.”

    2. Follow the steps to register. You’ll create your own user ID and password for SAW.

    3. The system will send you an email confirming your registration.

    4. Select the link in the email that follows this statement: “To activate your new account, select the following link.” This step is required to complete the registration process.

  2. Create a Link to the Department of Health Online Application Portal

    1. Login to your SAW account using your SAW user ID and password you previously created.

    2. Select the “Add a New Service” link.

    3. On the right column, select “Department of Health.”

    4. Scroll to “Online Application Portal.” Select “Apply.”

    5. You’ll be asked a series of questions based on your public record data (state of Washington doesn’t gather or store this data). The questions will match the name and address you provide with the existing public record information. You’ll want to ensure you’ve provided your full legal name and you may find you have to enter a former address, especially if your address has changed recently (within the past one to two years) to get the right questions that pertain to your identity.

    6. Click on “My Services” and choose “Online Application Portal.”

    7. Complete the information on the User Lookup Page, Last name, date of birth, and social security number (SSN) are required fields.

    8. If you don’t have a match, or have never had a state of Washington credential, select “No.”

    9. If you have a partial match, you’ll be asked additional questions. Select answer, then click “Validate.”

    10. If you have a full match, you’ll be asked to update any information that is currently incorrect. Once complete, click “Update.”

    11. You will now be able to choose the credential you would like to apply for. Select the credential from the list under “Complete a New Credential Application.”

    12. Please note, you will have 14 days from the start of the application to complete, pay for, and retrieve a copy of the application and any supplemental forms needed for completion.

    13. Pay the application and $2.50 convenience fee.

    14. After the payment has been successfully submitted, you can attach additional documents to your application at the “Upload Attachments” screen. If you have nothing to upload, choose “Skip.”

    15. The final screen will have a copy of any additional forms you may need to submit and a copy of the application you just finished.

Mail:

  1. Print and complete the License Application Packet.

  2. Print and complete the Credential Verification Form.

  3. Mail your completed application with initial documentation and your check or money order payable to: Department of Health PO Box 1099 Olympia, WA 98507-1099

After You Apply - What to Expect

  • Processing Time - varies for each application. It depends on how complete the application is, and how quickly the outstanding requirements are met. This could range from a few days to a few weeks, depending upon many factors such as the type of application, and whether it requires board or commission approval. Additional delays may be caused by:

  • An incomplete application

  • You submitted:

    • Incorrect or no application fee(s) as required

    • Outdated application forms

    • Electronically altered application forms.

  • Or, in some cases, we need additional documents such as:

    • Certified transcripts

    • Other education documents

    • Exam results

    • Certified court documents.

  • We'll notify you if the application is missing information or needs additional documentation. If you have questions on how to resolve missing information, documentation or other actions, contact the credentialing specialist listed on the notice.

  • Our review includes a thorough evaluation of basic healthcare credentials, employment or work history and any criminal or disciplinary history. In addition, out-of-state applicants must complete a fingerprint background check. This also increases processing time.

  • Please keep checking Provider Credential Search for a status change. This is the quickest and best approach to verifying your license status.

  • When can I start practicing?

    • Once your status has changed to "Active," you may begin working. You should receive your official document in the mail within 7-10 business days after your credential is issued.

Renewal

Dietitian licenses expire every year on or before the dietitian’s birthday.

How to Renew:

Online:

  1. Log-in to your account on the Secure Access Washington (SAW) Portal.

  2. Log-in to the Department of Health (HSQA) Online Services after logging into SAW.

    1. You’ll need the service code 7472 to access the HSQA Online Renewal System.

Mail:

Telehealth: State law requires all health care professionals, excluding physicians and osteopathic physicians, offering telemedicine services to complete telemedicine training.
