  • Allow 2 weeks for the Application to be processed once all items are received.

Renewal

Dietitian licenses must be renewed annually.

    1. You may send all completed, signed, and notarized application materials, along with your 12 required CEUs (PDP Log), and CDR Card with fee to email or address listed above.
