How to Apply & File:

Online:

  1. Completed Application - Create an account and complete online at MyLicense.IN.gov.

  2. Application Fee - $20 application fee to be paid by credit or debit card for applications submitted online. All application fees are nonrefundable.

  3. Positive Response Documentation -: If you answer "Yes" to any questions on the application, explain fully in a statement that includes all details. Include the violation, location, date, cause number, and disposition. Submit copies of court documents for each instance to support the statement. If malpractice, provide the name(s) of the plaintiff(s). Please upload at the time of application or log back into your account and use the License Update option.

  4. Name Change Documentation - Documentation of any legal name change if your name differs from that on any of your documents. Documentation may include a copy of your marriage certificate or divorce decree. Please upload at the time of application or log back into your account and use the License Update option.

  5. Official Transcript - Submit official transcripts from your college and/or university from where you received your degree. Please upload at the time of application or log back into your account and use the License Update option.

  6. Pre-Professional Experience - If you are registered with the Commission on Dietetic Registration (CDR), please submit a copy of a current registration card that indicates the period of registration and expiration. Please upload at the time of application or log back into your account and use the License Update option.

  7. Verification of State Licensure(s) - Verification of any registration/license/certification to practice any health-related profession or occupation in another state or territory. Verifications must be submitted directly from the state of issuance. Official electronic verification will also be accepted.

After You Apply - What to Expect

  • Processing Time - Allow 10 business days for review of online applications.

  • When can I start practicing?

    • You may not practice as a Dietitian in Indiana unless you are issued a license by the Board.

Renewal

Dietitian licenses expire on December 31st of even-numbered years.

Telehealth:

The Telehealth Certification for Out-of-State Practitioners is required by Indiana Code 25-1-9.5-9. This certification is required to be completed by an Indiana-licensed practitioner who is physically located outside of Indiana and intends to establish a provider-patient relationship or issue a prescription for an individual located in Indiana.

Online:

  1. Please use the “Personal Licensing” option.

  2. If you have an existing Access Indiana account, select login using your existing account.

  3. If you do not have an existing Access Indiana account, click “Register for an Access Indiana account” and then use the Person Search Function.

  4. You can enter your DOB and SSN to search for an existing personal record.

    1. If a record is found, your information will be populated and then click Register.

    2. If no record is found, you can enter your information and click Register.

  5. Click Initial Application on the left-hand menu.

  6. Select Telehealth Certificate.

  7. Follow the prompts to receive your certification.
