1099 Tax Form
Berry Street issues 1099 forms to all independent contractors earning $600+ annually. These forms are sent via secure email by January 31st for the previous calendar year.
Example: If you began seeing patients in December 2024 and received your first payment in January 2025, your first 1099 will be issued in January 2026 (for income earned in 2025).
