1099 Tax Form Berry Street issues 1099 forms to all independent contractors earning $600+ annually. These forms are sent via secure email by January 31st for the previous calendar year.
Example: If you began seeing patients in December 2024 and received your first payment in January 2025, your first 1099 will be issued in January 2026 (for income earned in 2025).
W-9 Form (Required) To generate your 1099, Berry Street requires a W-9 form at onboarding. Information collected includes:
Name: Personal or business
Address: Residence or business location
Federal Classification: Individual, LLC, etc.
TIN: SSN (individual) or EIN (business)
