This section outlines how to coordinate patient care through family communication, care team collaboration, referrals, and documentation.

This section provides guidance on coordinating patient care through effective communication with families, care teams, and external providers, including account setup for dependents, sharing information securely, managing referrals, and handling documentation requests.

Creating Patient Account for a Child without an email address

  • If you have a patient who would like to sign a dependent up for a session and use their own email address, you can create a new account with an extension of the parent’s base email.


Collaborating with a Patient’s Care Team

When a patient provides a Release of Information (ROI) for another provider (e.g., therapist or physician), use it to establish communication and foster coordinated care. Send an introductory email with the ROI attached to open a line of collaboration and support better outcomes and potential referrals.


Faxing with External Care Team Members

For Berry Street–Sourced Patients

  • Sending a Fax: Email the documents and full recipient details to [email protected].

  • Clinical Notes: Include the date of service, and our team will fax it for you.

  • Receiving a Fax: Ask the sender to include your name and the patient’s details so Berry Street can notify you and upload it to the chart.

For Provider-Sourced Patients

If you self-source patients, we strongly recommend setting up a HIPAA-compliant personal fax (e.g., (FAXAGE, SR FAX, eFax).

  • Any faxes sent to the Berry Street fax number will be treated as Berry Street–sourced.

  • Berry Street does not automatically fax Clinical SOAP notes for provider-sourced patients.


Referrals for MNT Services

Berry Street handles referral outreach for both Berry Street (BS) and Provider Sourced (PS) patients when:

  1. Insurance requires it for coverage

  2. Additional appointments need approval

  3. Licensure requires a referral (Berry Street Medical Director provides it when needed, ex. for CA patients)


Patient Requests for Documentation and Medical Letters

Patients may need letters or documentation for:

  • HSA/FSA usage

  • Product recommendations (e.g., supplements, medical testing)

  • Clinical chart notes for care coordination
