Coordinate care for family members and partners by leveraging shared coverage, following age-appropriate engagement guidelines, and ensuring clear billing practices under the correct patient account.

Key Point: Family members of Berry Street–sourced patients are classified as Provider–sourced. Review family dynamics to maximize nutrition coverage for the whole household, and let parents or guardians know that dependents and spouses on the same plan often share coverage.

Dependents by Age
0–5 yrs: Child does not need to be present. Account should be created under the child’s name.
6–10 yrs: Similar to 0–5; parents may choose to include the child briefly. Account under child’s name.
11–12 yrs: Begin including the child as appropriate. May meet with parent/guardian only or include the child for part of the session. Account under child’s name.
13–15 yrs: Offer sessions with parent/guardian present for all or part of the session (e.g., first/last 5–10 minutes). Supports trust, autonomy, and communication.
16–18 yrs: Encourage more 1:1 time with the teen. Parents can join briefly or receive a session summary via email with the teen cc’d. If appropriate, teens may attend full sessions independently. Most have their own email; parents can be set up to receive reminders and invoices without PHI access.

Partners
Sessions with both partners are billed under one patient's Healthie account; no double booking.
Couples’ sessions are not billed as groups unless 8–10+ participants.
Use individual 1:1 sessions first, then integrate with joint sessions.
If one partner is Berry Street–sourced and the other Provider–sourced, bill under the PS patient when possible.
Document the primary patient vs. supporting partner clearly for billing accuracy.
Creating an Account for a Child without an email address
If you have a patient who would like to sign a dependent up for a session and use their own email address, you can create a new account with an extension of the parent’s base email.

Sending a "Consent to Treat a Minor for MNT Services" Form
When working with any patient under 18 years old, providers can obtain a signed "Consent to Treat a Minor for MNT Services" form from a parent or legal guardian before beginning care. This form ensures compliance with state and federal regulations, as well as Berry Street policy for treating minors.
When to Send the Form
Anytime you plan to meet with a patient under 18.
When a minor transitions from parent-led sessions to partial or full 1:1 care.
If a new dependent joins your caseload through a parent or guardian account.
How to Send the Form Providers can request the form through Berry Street Support:
Email [email protected] with the subject line: “Minor Consent Form Request.”
Include the following details:
Patient name and ID
Parent/guardian name and email
The Berry Street team will send a secure, electronic form for signature and notify you once it’s completed.
Providers can trigger a form to be sent directly to the parent/guardian:
Navigate to the patient chart.
Click "Charting" → "Request Form Completion" → "Consent to Treat a Minor for MNT Services"
Click "Send Form Request Email"
