Insurance companies or third parties may occasionally contact you directly (as the rendering provider) to request medical records or “verification” for a Berry Street patient—or a patient in the Berry Street system (including patients from prior settings or provider sourced patients) that they believe is associated with Berry Street. Do not ignore these requests. Please route them to Berry Street so we can respond appropriately, meet deadlines, and ensure compliance.

  1. Do not send records yourself.

  2. Forward the request to Berry Street immediately:

    1. If the request is by email or fax: forward it to [email protected]envelope (our team will route it to the correct internal owner).

  3. Tell the requester to contact Berry Street directly for records.

Steps when you receive a request

If you receive an email request: Forward the entire email thread (and any attachments) to: [email protected]envelope.

If you receive a phone call: Collect the call-back number, reference/case ID, payer/organization name, patient name + DOB (if provided), what they’re requesting, and any deadline. Then email these details to [email protected]envelope.

If you receive a request by mail (snail mail): Scan all pages (even if you don’t think it’s necessary) or take clear photos of every page, and forward them to [email protected]envelope.

Important Things to Note

  • Some requests are sent in error (for example, the patient may not be a Berry Street patient, or the payer/third party may have incorrect information on file). Forwarding the request to Berry Street helps us verify the member and the request details quickly.

  • Timely routing matters for compliance. Medical record requests often trigger clinical note/chart reviews, which are a required step for insurance network compliance and may have strict deadlines.

  • When in doubt, forward it. If you’re not sure whether the request is legitimate or relevant, send it to [email protected]envelope anyway—our team will triage and take it from there.
